The Vacancy
North Lincolnshire Council are looking for an experienced and self-motivated case worker to work within our Learning Disability and Sensory Work Team. This is a full-time position. You will be required to have experience of working directly with those who have a Learning Disability and/ or Mental Health needs.
You will be a highly motivated case manager with experience of undertaking assessments, support planning, commissioning, and safeguarding. You will have a detailed knowledge of adult care and its underpinning legislation. You will need to provide person centred, strength-based assessments and care plans which focus on maintaining independence. You will need to be able to work autonomously and demonstrate excellent problem-solving skills. You will need to have effective communication and organisation skills as well as the skills in negotiation and use of IT systems and the ability to work under pressure.
Key responsibilities include managing a caseload of complex cases. You will also participate liaising with other teams, both internal and external, to ensure a good quality service that is in line with our statutory responsibilities and established good working relationships.
