Healthy Lifestyles Facilitator - 18.5 hours

The Vacancy
Are you looking for a new opportunity to make a difference to the health and wellbeing of people in North Lincolnshire?
Working within North Lincolnshire’s Healthy Lifestyles team, you will be joining two Healthy Lifestyle coordinators and a team of facilitators; you will provide efficient information and support to groups and individuals who want to make positive changes to their health and wellbeing.
Predominantly this role will focus on reducing smoking prevalence in North Lincolnshire with additional focus on obesity rates subject to service need. Also contributing to wider health and wellbeing needs through NHS Health Checks.
To be a successful Healthy Lifestyle Facilitator you will:
- Be passionate about making a difference to the lives of individuals in North Lincolnshire
- Have experience of working with individuals to bring about positive behaviour change
- Have excellent communication skills
- Be adaptable in your approach dependent on your audience
- Use a collaborative approach
- Be able to effectively prioritise workload and keep the digital case management system up to date
- Be adaptable to meet the variety of requests and variable workload
- Have experience of working with a broad range of stakeholders and building positive relationships including GP Practice, midwifery and social prescribers
- Have a background in one or more of the following: nutrition, physical activity, public health, community health and adult weight management
- Be an excellent team player and have a willingness to learn and develop
