This is a varied, people‑focused role combining direct support, community engagement, and dementia awareness.

A little bit about the role

This 12 month role has been created to improve awareness, referrals, early diagnosis and access to support for people affected by dementia from minority and rural communities.

As a Dementia Community Engagement Worker, you will lead on developing culturally responsive outreach, education and partnership work, tailored to the specific needs of rural and minority communities across North Lincolnshire. Your work will lead to more people from these communities living with Dementia accessing support and receiving diagnosis.

You will act as a trusted bridge between people living with dementia, their Carers, community organisations, and primary and secondary care services, helping to identify barriers to engagement and working collaboratively to overcome them.

A key part of this role is listening to communities, amplifying lived experience, and ensuring dementia services are inclusive, accessible and relevant.

Your work will help people feel understood, respected and confident to access support at the right time.
