The Vacancy

The Older People and Physical Disabilities Social Work team are looking to recruit 2 full time social workers to join our existing team. We are a longer term social work team working with adults aged 18 plus, this means that our work is varied and changeable and covers individuals with a variety of different needs.
Within your role you will be required to provide long term case management, this will include undertaking statutory reviews, care planning and multi agency working with people in their own homes and care homes.
The right candidate will be someone who has a social work degree and who are registered with Social Work England. They will be a strong team player with a proactive approach to work as part of a busy Social Work Team.
The successful candidate will be highly motivated and willing to undertake Care Act Assessments, support/care planning and commission services with person centred outcomes. They will be able to provide social work supervision to unqualified members of the team. They will be able to travel throughout North Lincolnshire independently as we cover all of the North Lincolnshire area. Knowledge of adult social care legislation and frameworks are essential.
You will need to demonstrate an ability to problem solve with good negotiation skills, along with effective communication, organisational and IT skills. You will need to be flexible, resilient and dedicated to team work.
